Apple SSD does not show in drive's list


the problem: ssd not available not in os x nor bootcamp win8.1

situation: in october 2014 had upgraded late 12 imac 27 apple's original 256gb ssd , rebuild storage system fusion drive. in feb 2015 after parallels update mac frozen , fusion drive in offline status after reboot. when check disk utility shows 1tb (hdd capacity). found if had peel off sata cable hdd , start system without ssd become available through disk utility. when turn hdd cable , reboot fusion drive rebuild no loss of data. february till april there 5-10 times when repeat procedure after freezing. made decision remove fusuion drive, check smart status of both hdd , ssd , secure erase drives.
did of data time machine
have removed corestorage volume
checked smart status - perfect both drives.
proceed secure erase on hdd - ok!
start secure erase usb stick 10.10.3 osx setup , mac freeze again @ 20-30%

, after reboot there no ssd in os x.
have make bootcamp partition , install windows 8.1 on it.
in both sata channels device manager showed apple hdd , no name, drive manager did not show except hdd
got acronis disk director tryout , check drives with no result.

looks windows know there on sata 1 channel not mount because of problem.
osx disk utility , disk warrior both show nothing ssd

suggestions how solve problem?
