mismatched button icons in multiple programs after update

i upgraded yosemite, , far works fine. have noticed there buttons have incorrect graphics on them. biggest 1 i've noticed happens on lot of programs such safari, , system preferences window.

back/forward icons in upper left. instead of left , right arrow expected icons. button has forward icon on it, , forward button has lock icon on it. instance of sidebar button in safari. instead of standard sidebar icon, showing refresh icon.

buttons work should, icons incorrect. has ever seen issue before, , know can fix it?
 

just update this. after reading multiple other posts on other sites have tried clearing cache folders in main library folder, , user accounts library folder , rebooting , has not helped.

went disk utility , repaired permissions , rebooted in case fix it. still no go.

have other thoughts how fix this?
