iPhone 5 unable to connect to network

i have iphone 5 running 8.2. sitting @ work other day when phone randomly disconnected network. when tried reconnect network tell me "unable connect". tried connect again , said same thing. here have done:

1. forget network. - tried connect afterwards , still unable connect.
2. reset network settings - when try connect says our network enterprise network , wants know username , password. our network not enterprise network. other people can connect network no problems.
3. factory reset phone. - same problem upon restore.
4. reset router, modem, , things pertaining internet. else can still login no problem mine says unable connect.
5. have clicked "other" , typed in network name , password. "unable connect" box again.

nothing seem helps. dont know why phone thinks enterprise network. beyond confused.

iphone works on other networks try connect to. 1 network , no 1 else has problems. have tried every combination on youtube, forums, etc... cant find else has problem.
